Background Check Authorization

By electronically accepting this authorization, the applicant authorizes Eagle Eye Servs LLC and its authorized screening providers to obtain and review legally permissible background information for driver qualification, safety, compliance, identity verification, and ongoing eligibility purposes.

This may include driving history, criminal-history information where legally permitted, identity verification, and other records relevant to the services performed. Eagle Eye will use screening information only for legitimate business and compliance purposes and in accordance with applicable law.

The applicant certifies that the information provided is accurate and understands that additional notices or disclosures may be required by law or by a screening provider.

Electronic acceptance has the same effect as a handwritten signature to the extent permitted by law.
